FIRST APPOINTMENT INFORMATION

Your first appointment will be scheduled for one hour.  If for any reason you can not make the first scheduled time, please contact us so that we may reschedule your appointment and allow another patient on our waiting list to utilize that time.

It is important that you arrive at least 20 minutes prior to your appointment, so that all the required electronic chart information can be completed, thus your entire appointment time with the doctor can begin on schedule.

Upon arrival you will be required to present your insurance card (if we are billing your carrier) and picture ID. You will have an option to use our lobby kiosk to enter your health history and current condition information.
